Most odd mod combination i found? The silencer for shotguns! But quick online search shows they actually exist!
Another odd combination is bow/crossbow structural brace mod which states it's for melee weapons and tools
Many interesting mods compatible with the Robotic Sledge
 
Bugs i discovered while making this list:
The Trigger Group: Burst Mod goes to tool belt instead of inventory first unlike all others mods
 
Mods in tool belt do not highlight usable until you pick one up off tool belt, then all usable tool belt items start highlighting properly. It will even show incompatible items compatible on tool belt when switching between modding items until picking up an item on tool belt to refresh
 
Please let me know if i missed anything, Longer then i expected to put this together, i spent 5 hours on this today and started going crazy, most likely missed something. I probably could have presented this differently with less repetition but i wanted a way to look up specific item and immediately see a list of all mods and descriptions. Also order is kind of random, as a result of how i was loading all mods in my inventory to see which are compatible or not.

I was going to ask you how you did this. Lot of work, good job. If you're interested I can attempt to auto-extract the mod compatibility from the XML files to compare with your data. Or you can play with it yourself if you want to get into the XML. What you're looking for is the "tags" attribute on items in items.xml:
 

    <item name="meleeToolAxeT1IronFireaxe">
        <property name="Tags" value="axe,melee,grunting,medium,tool,longShaft,attStrength,perkMiner69r,perkMotherLode,canHaveCosmetic"/>

 

And then compare it with the installable_tags on the mods in item_modifiers.xml:

 

    <item_modifier name="modMeleeTemperedBlade" installable_tags="perkDeepCuts,axe,shovel,perkBrawler,perkJavelinMaster"

 

If you get a match, the mod will work with the item. A little text searching and you can get 100% of the matches for certain hopefully without manually adding/removing mods in-game!
