I think it's because "semi truck" is already a different block. Semi Truck has wheels behind it whereas Semi Truck Modular Front does not

No worries, I actually made a report here on this exact same issue not too long ago without realizing it was a known issue xD
Only mods/staff can move the threads. Looks like this one is already moved to the duplicates section

I might have a repro for this
- Rent a vending machine
- Place one full stack of lead into vending machine
- Fill up entire inventory with junk except for 1 empty slot
- Put 3 pieces of lead in empty slot
- Attempt to retrieve all 6000 lead from vending machine
- Observe remaining lead is 3
- Close vending machine and make inventory space
- Open vending machine again, observe the game is trying to retrieve 10 lead but you only have 3 in vending machine
- Try to take 10 lead out
- Machine now says -7
- Log out
- Log back in
- Check vending machine
You now have 65529 lead 😅

Logs might be useful here, as some game settings will put your game in the "modded" category and not qualify for achievements (for example enabling creative mode or increasing the XP multiplier).

It won't help remove existing markers but at least it won't keep adding new ones

There was a similar report here. Did you perhaps have a game that was on 21.0 and then upgraded it to 21.1?

Splints and Plaster Casts are only for broken arm or leg. They won't help if it's sprained. Best thing for sprained limbs is to take steroids so it doesn't get worse, but based on chiko's list you can't just stab your teammate with steroids so they will have to take it themselves 😋
Oh nevermind, I forgot that physician perk can heal sprains now. Disregard 😅

So I was able to repro on a new save, but found that it's a bit tricky to repro. It feels like it really depends on the map/location too, as I was not able to repro on Navezgane but was able to repro again on a new save of Pregen6k
One thing I did notice was that it requires at least 1 relog for the issue to happen (so set up the lights, teleport away and back, see the lights are still normal, then relog and teleport away and back again).
If that still doesn't work, I've tried best I can to make as specific of a repro as I could of exactly where I placed the lights and where I teleported
- Create new pregen6k map
- enable DM and CM
- In the console, type "tp -70 35 1053"
- There should be a light pole nearby, place the battery bank in front of it and power with car battery and turn it on
- Place down 9 Industrial Light bulbs, 1 in front and 4 on left/right of the bank
- Wire up the bulbs
- In the console, type "tp -2689 148 -2679"
- Wait a bit (for trees and stuff to start appearing)
- In the console, type "tp -70 35 1053"
- Lights should still look normal
- Log out and back in
- enable DM and CM
- In the console, type "tp -2689 148 -2679"
- Wait a bit (for trees and stuff to start appearing)
- In the console, type "tp -70 35 1053"
Some of the lights should start being different colors now

I see, if you are admin or host and comfortable using debug commands, try this
- Go to the POI (so that the POI name shows up on the top right). Preferably just outside the fence or something
- Hit F1, a console should pop up
- Type in DM and hit enter to go into debug mode
- Type chunkreset f and hit enter. This will reset the POI blocks but not respawn any zombies
- Now to fully reset POI, close the console (hit esc), then hit Shift + F6 and it should pull up a menu
- Search for test_POIReset and click on it. This will respawn the zombies
To turn off debug mode, just repeat step 2 and 3

Forgot to ask - Is there a way to fix it, or is that POI permanently bugged for me now?
Try removing the chest and start quest again maybe?

I've looked at both mine and your error logs and it seems to be the mod "Improved Hordes" that seems to be causing all of the errors and maybe even the block-upgrade problem
You need to talk to the person who made the mod that is causing the problem. Most likely they need to fix something in their mod and provide an updated version.

Just wondering, is your lv 4 bow equipped with the same type of arrow as your lv 5 bow? I've noticed if you equip different arrows it updates the stats of the bow. For example exploding arrows on a lv 4 bow will show that it's much much stronger than a lv 5 bow.

Nevermind. I think the issue here is I'm giving myself quests using debug commands (ex: givequest test_sleepers), if I get a quest normally through a trader, it allows me to start the quest over when I relog

Unfortunately I don't know exact numbers but I have seen this behavior in a couple blood moons now. I would say usually more than 5 players are on but less than 10. If I see it happen again I'll take note of number of players

Update: I was able to get a clip of it happening in SP as well, but as mentioned it's very hard to get consistently here is video from the moment I made a new map

Are you trying to read a book or a magazine? Books come in volumes 1-7 and only gives bonus once, reading same volume again just gives you xp. Magazines on the other hand needs to be read multiple times to improve crafting skill.
When you said you read spears, is that "spear hunter" (the book) or "sharp sticks" (the magazine)?
There will also be an icon on the top right of the book/magazine. Open book icon means you've completed it already and will only get xp from reading it.
