I generally keep an eye on whether I got xp from them to make sure they are dead-dead.
You mentioned the prison, one of the prisons actually does have triggers that will spawn zombies behind you, in areas that you've already cleared (it's a new mechanic/design in A21 from what I understand), so perhaps that could add to the confusion?
