On a20 (b238) stable that was just released I ran a T4 "clear" quest on Couthouse_Med_01 and I got the quest completion status after killing 2 zeds in the small attic section before getting to the final loot room up on the roof where 3 zeds proceeded to drop from the ceiling. I just ran this the day before on EXP and it worked fine. I don't know if this is relevant, but did note, you also changed the layout of the jail cells from EXP to Stable so those changes may have had an additional impact to the POI.
