  11. Looking at it this morning, found that you don't need to enter the text at all. Just swapping from one item to another without using it causes the item to shrink or grow. You can also easily see this on yourself by entering 3rd person mode

  17. Hmm. Try closing your camera and doing it again. I got it to work off of your screenshot even. On my iPhone 5S, the camera doesn't like it if it's already open. On this phone, I don't have a QR code app. On my iPhone XR, the app will pick it up every time. I feel like this is more a hardware issue than the actual QR code itself.
